Today we will look at passport requirements for those who want to renew their passports after getting a passport appointment schedule. 🙂
Kung may lumang passport ka na, pwede mo na palitan ng bagong electronic passport yan.
Requirements for passport renewal
Brown Passport or issued prior to 01 May 1995
- Old passport and photocopy of passport pages 1,2,3 (amendment). The pages showing latest Bureau of Immigration departure and arrival stamps.
- Supporting document with complete middle name.
MRP & Green Passport or issued after 01 May 1995
- Present passport and photocopy of inside and back cover.
- The pages showing latest Bureau of Immigration departure and arrival stamps.
- For illegitimate minor, personal appearance of mother is required.
For married women, who want to use surname of their spouse:
- Personal Appearance is required
- Marriage Contract (MC) in Security Paper issued by NSO or CTC issued by the Local Civil Registrar duly authenticated by NSO.
For Minors (below 18 years old): Personal appearance of either parent is required.
- If minor is NOT traveling with either parent:
- Original and photocopy of DSWD Clearance
- Affidavit of Support and Consent
- If both parents are abroad:
- Affidavit of support and consent (must be authenticated by the nearest Philippine Embassy or Consulate General if not executed before a Philippine Consul).
- Special Power of Attorney (must be authenticated by the nearest Philippine Embassy or Consulate if not executed before a Philippine Consul designating the representative by name and authorizing him to apply for a passport on behalf of the minor).
- Passport and photocopy of the passport of person travelling with the minor.
If new passport applicant ka, here are the requirements.
Passport appointment can be acquired by calling 7371000 or logging in to the website www.passport.com.ph

Here is the complete list of IDs and documentary requirements for first time passport applicants:
GENERAL REQUIREMENTS
• Personal appearance
• Confirmed appointment
• Duly accomplished application form – may be downloaded from the DFA website, http://www.dfa.gov.ph
• Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable. (REPORT OF BIRTH DULY AUTHENTICATED BY PSA IF BORN ABROAD)
• Valid picture IDs and supporting documents to prove identity (Please refer to List of Acceptable IDs and List of Supporting Documents)

Nagkamali po ako ng lagay ng place of birth sa passport application form, ang nalagay ko yung lugar kung saan ako bininyagan pero tama naman po yung nakaindicate sa NSO ko. Okay lang po ba yun? Pwede ko ba ipabago nalang. yun dun sa mismong sched ko sa dfa. And kailangan din ba latest yung NSO pag pinakita? Actually last year lang tong NSO ko, need ko pa ba iparenew ulit for this year? Kung readable at nasa maaayos pa namang conditionang dokumento mo pwede pa yan sa DFA. Yung form na prited out na hawak mo pwede mo na baguhin yung details dyan.

Hello po. Ask ko lang po. Anu pwd gawin pra mkkuha ng passport. Kc po Late Registered po aq sa NSO. Bali illegitimate po ako. Walang tatay na nakalagay sa NSO cert. Ko Pero my nkalagay n middle name at sa nanay ko un. Lumalabas po n prang kapatid kami ng nanay ko. . Meryll Relano Cajandoc po ako at nanay ko Anecita Relano Cajandoc..

Pwede ka naman mag apply ng passport kahit ganyan ang details mo sa PSA(NSO) copy mo eh. Follow PSA details naman ang DFA at meron ka naman atang mga supporting documents.

Basic requirement ang IDs kahit for passport renewal. Kahit isang ID lang sa mga sumusunod ang dalhin mo:
Government-issued picture IDs such as the following:
Digitized SSS ID
Driver’s License

Hindi. Kung saang DFA Office yung pinili mo doon lang valid ang appointment na yan.

Yung appointment online na ginagmait ngayong DFA ay para sa Manila offices lang. Yung mag provicial DFA offices may sariling silang system kung may appointment silang binibigay.

Ano ba talaga yung reson ng pagkawala yun ang ilagay mo pwede ka pa naman pagawa ng police report eh. Kapag meron ka na nung dalawa na yan punta ka ng DFA Aseana para maka pag file ng lost passport.

Ok lang naman ang mbuhok na may kulay pero hwag yung loud colors. Kung pwede sana after mo nalang mag renew ka magkulay 🙂

Ngayon nyo lang nakita na mali yung year of birth mo sa passport? For renewal na yan. mag set ka ng appointment at magdala ka ng NSO B.C mo at mga I.D na mag papakita ng tamang birth date mo.

Ask ko lang kung pwede ko pang gamitin yung mga id’s and passport ko nung single pa ako sa pag apply to Uk kasi hindi pa naman expire tsaka pag mag renew ako matatagalan at lahat ko babaguhin pwede kaya yun?Bago lang kasi kaming kasal ng husband ko kaya di ko pa naayos ang pag chichange ng surname ko to my husband surname. Depende sa application mo kung i de declare mo na single ang status mo hinid ba yun makakaapekto?
Yun nga po inaalala ko MasterCitizen kung makakaapekto kasi may friend akong may asawa din na foreigner pero yung ginamit nyang document is nung single pa siya, sabi niya ok naman basta may marriage certificate ka na married ka, eh kaso nadeny yung application niya pero hindi naman sinabi na isa yun sa dahilan kung bakit di siya nakatanggap ng visa.
Yun ang problem kasi nka declare ka na married tapos wala kang M.C na ipapakita diba.

Pwede mo na irenew yan. Regarding sa policy sa DFA Lucena hindi ako sure kung by appointment sila.

Yes maganda na yung hawak mo na mga documents. Kung sa DFA manila branches ka mag pa file dapat may appointment sked ka muna bago pumunta sa mga offices nila ha.Good luck 🙂

Yes ok na ang mga yan. All you need na lang is an appointment schedule if you are going to renew it in DFA manila branches.

Normally hindi pinapayagan. Humanahanap ng government issued ID. Pero kung bata ang applicant na galing sa school, pwede ang school ID
