The DFA Passport Guide

Bonjour. Mabuhay.

If you are planning on getting a passport, this short guide will help you get through the basic steps of preparing, applying, and completing your passport plans. A 17-step guide on the intricate but simple details of getting yourself ready and able for your plans of going places.

Here are the  details:

1.

Get a passport appointment schedule.
  • – Log-on to www.passport.com.ph.
  • – Click the “Set an Appointment” button.
  • – Choose the DFA Office where you want to set an appointment.
  • – Fill up the application form completely.

2.

Confirm your appointment and print your application form.
  • – Check your given e-mail address for DFA passport appointment confirmation message.
  • – Click the confirmation link within 24 hours to confirm the appointment otherwise the appointment will be cancelled.
  • – Print the application form using a PDF reader.

3.

Complete the necessary requirements and have them photocopied
  • – Ready all the necessary requirements as stated in the checklist with their photocopies.

4.

Personal appearance on the date of appointment and present the printed application form.
  • – Submit the printed application form together with the requirements on the appointment date.

5.

Requirements in case the applicant has no birth record:
  • – Apply for a delayed registration of birth at the local civil registry at the place of birth of applicant.
  • – Submit late registered NSO Birth Certificate with supporting documents.

6.

Additional requirements for applicants whose birth was late registered.
  • – Form 137
  • – Voter’s Registration Record
  • – Baptismal Certificate or National Commission on Muslim Filipinos/Office on Muslim Affairs Certificate with dry seal.

7.

Additional requirements for married women.
  • – Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable.
  • – Report of Marriage duly authenticated by NSO if married abroad.

8.

Additional requirements for spouse of foreign national:
  • – Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable.
  • – Report of Marriage duly authenticated by NSO if married abroad.
  • – Original and photocopy of Commission of Filipinos Overseas (CFO) Guidance and Counseling Certificate of Attendance (required for first time applicants or renewal of passport to be used for the first time the surname of the husband).

9.

Requirements for applicants with foreign sounding names:
  • – Bureau of Immigration Identification Certificate
  • – Certificate of Naturalization
  • – Election of Filipino Citizenship/Oath of Allegiance.
  • – If necessary, Marriage Certificate of parents and Birth Certificate of applicant’s brother or sister issued by NSO.

10.

Requirements in case the applicant is minor:
  • – Confirmed appointment (except for 1 year old and below).
  • – Personal appearance of the minor applicant.
  • – Personal appearance of either parent (if minor is a legitimate child)/ of mother (if minor is illegitimate child).
  • – NSO Birth Certificate
  • – Document of identity with photo, if minor is 8-17 years old (for first time and renewal applicant) such as School ID or Form 137 with readable dry seal
    • For minor applicants who never attended school, a Notarized Affidavit of Explanation executed by either parent (if minor is a legitimate child) / by mother (if minor is an illegitimate child) detailing the reasons why the child is not in school, is required.
  • – NSO Marriage Certificate of minor’s parents.
  • – Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child).
  • – Original and photocopy of valid passport of the person traveling with the minor.
  • – Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) or identification documents.

11.

Additional requirements in case minor is not travelling with parent:
  • – Personal appearance of the mother is required if minor applicant is an illegitimate child.
  • – Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion and relationship to the minor. If minor will be traveling alone, ASC must be executed by either parent, stating that his/her child will be traveling alone. If minor is illegitimate, mother should execute the ASC.
  • – Original and photocopy of DSWD Clearance
    • There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.

12.

Additional requirements if both parents of minor are abroad.
  • – Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion (authenticated by the nearest Philippine Embassy or Consulate General). If minor is illegitimate, mother should execute the ASC.
  • – Special Power of Attorney (SPA) with an attached photocopy of either parent’s valid passport (authenticated by the nearest Philippine Embassy or Consulate General) authorizing a representative in assisting the child to apply for a passport. If minor is illegitimate, mother should execute the SPA.
  • – Original and photocopy of DSWD Clearance
    • There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.
  • – Proper ID of the duly authorized representative.

13.

Requirements for minor whose mother is also a minor.
  • – Personal appearance of mother and maternal grandparent/s
  • – NSO Birth Certificate of minor applicant and mother
  • – Affidavit of Support and Consent executed by the maternal grandparent/s indicating the name of the traveling companion
  • – DSWD Clearance if minor will be traveling with the person other than the maternal grandparent/s
  • – Proof of identity of mother and maternal grandparent/s.

14.

Requirements for Muslim applicants.
  • – Personal appearance
  • – Confirmed appointment
  • – NSO Birth Certificate
  • – Valid picture IDs and supporting documents to prove identity.
  • – Certificate of Tribal Affiliation from the National Commission on Muslim Filipinos (NCMF) if late registered.

15.

Requirements for converts who would like to use their muslim name:
  • – Annotated Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) bearing the Muslim name
  • – National Commission on Muslim Filipinos (NCMF) or Office on Muslim Affairs (OMA) Certificate of Conversion

16.

Additional requirements if applicant is a DOST Scholar.
  • – Certification or clearance from DOST.

17.

Requirements for applicants who want to avail a Dual Citizenship.
  • – Identification Certificate
  • – Oath of Allegiance
  • – Order of Approval
  • – Report of Birth (for those born abroad)

Salamat sa mga info Master,, talagang informative , will surely share this blog to others..Mag tanong po sana ako para sa passport application ang valid Id ko lng po kasi ay NSO birthcert at sss id yung drivers license po kasi hindi pa card type… pwede na po ba yun? thank you nakapag pasched na rin ako online sa DFA.

Gud day po…ask ko lang po firstime ko po to get passport pwede na po ba itong mga requirements na to voters I’d,philheat I’d,form 137,baptismal, married cintract nso,birthcertificate nso,nbi ko po resibo palang baka hindi na po kasi umabot sa date.. Ask ko po kung makakakuha ako gamit yan mga papers..thx po;)

      Base sa passport.com.ph ito ang mga general requirements para sa first time application. Sundin mo ito para sigurado:

      GENERAL REQUIREMENTS

      1. Personal appearance
      2. Confirmed appointment
      For NCR-based applicants, present your printed application form downloaded from DFA Appointment System
      3. Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable. (REPORT OF BIRTH DULY AUTHENTICATED BY PSA IF BORN ABROAD)
      4. Valid picture IDs and supporting documents to prove identity.

      LIST OF ACCEPTABLE IDS (At least 1 of the following):

      Government-issued picture IDs such as the following:
      Digitized SSS ID
      Driver’s License
      GSIS E-card
      PRC ID
      IBP ID
      OWWA ID
      Digitized BIR ID
      Senior Citizen’s ID
      Unified Multi-Purpose ID
      Other acceptable picture IDs such as the following:
      Old College ID
      Alumni ID
      Old Employment IDs

      LIST OF SUPPORTING DOCUMENTS (Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant, At least 2 of the following):

      PSA Marriage Contract
      Land Title
      Seaman’s Book
      Elementary or High School Form 137 or Transcript of Records with readable dry seal
      Government Service Record
      NBI Clearance
      Police Clearance
      Barangay Clearance
      Digitized Postal ID
      Readable SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
      Voter’s Certification, List of Voters and Voter’s Registration Record (please attach receipt)
      School Yearbook

  5. Good afternoon po ask ko lng po about sa passport kung included po ang saturday and sunday sa working days and ilang days po ang pinakamabilis ksi po sa blog nyo 7days then sa dfa website 10 days.

      1. Sa june 10 po ksi ang appointment ko.inistimate ko polung anung day ko marerelease ang passport ko pra pra mareschedule ko yung flight ko.

      2. May minor damage po ksi ang passport ko natuluan ng tubig ngkawater mark dun sa biometric part dun sa taas where the republic of the philippines five letter REPUB ang my damage baka madeny po ako dto sa korea khit sofa visa holder ako

      3. Basta na damage subject to renew na nag passport eh. Kung aabot ang flight sa release ng passport mo much better kung hind for re booking ka na.

  9. pd po bang magpabalik balik ng bansa kapag multiple entries at sa january 2015 pa ang expiration ng visa? i mean pwede po umuwi sa pinas ngaung october to november? nakapunta n kmi sa barcelona for residence kaso may emergency na uuwi kmi sa pinas ulit for 15 days lang. so wla nmn pong problema sa visa dba as long as january pa ang expired , multiple pa.

  12. hi sir 🙂 nakapag-apply na po ako kanina ng passport sa dfa. sa september 10 na irrerelease. kaso po, nagkaproblema sa first name ko at inhonor nila yung nasa birth certificate ko. ESTRELVIRA po yung nasa NSO while ESTREL VIRA naman ang gamit ko sa mga documents. ano po kayang maeencounter kong problem regarding this.? sabe po kase sa dfa, baka ma-question ako sa immigration. at may flight na po kase ako sa October. pano po kaya iyon.? thanks po.

    1. Ano ba yung alis mo work or tour lang? Magdala ka ng affidavit of discrepancy and some identification na with pictures and full name mo as proof na iisang tao lang yan. Pero ipa correct mo na din yung NSO mo para maayos mo and in yung passport mo the earliest possible time na kaya mo.

      1. tour lang po. pano po ako makakauha ng proof of discrepancy? pag pina-correct ko po ba yung NSO BC ko, papalitan din yung name sa passport.?

      2. Affidavit lang yung sinasabi ko para may proof ka na one and thesame person lang yung nasa passport at ikaw. Kapag naayos mo na yung NSO mo ipa pa renew mo na yung passport mo para magamit mo na yung tama.

  13. what do i need to do to get the passport of a child when the mothers where abouts are unknown? the parents are not married. the father wishes to take the child to his jhome country but cant as no passport no mother to do the paperwork

      1. in the mean time the child is stuck in the country until the father files for legal guardianship.
        and would you know how long this takes? can the PAO office assist?

  14. gud day sir/ma’am, i would like to know if can i apply to agencies after i get passport if i hired. I hev no idea for working abroad especially for the requirements. Do i need get passport before i apply. How many years the passport will ended?

    1. Passport validity is 5 yrs you can see in the page where your picture is. Yes it’s much better that you have a passport first before you apply for work abroad. It’s better to have an agency just make sure it is a legal or licensed agency. poea.gov.ph for list of accredited agency

  15. Iba pa po ba yung passport applications for students, immigrants, OFW’s and tourist? kasi may reporting po ako about dun. e wala naman po akong makita na iba pa yung requirements na need nila. thanks po 🙂

    1. Walang pinag kaiba yun for general purpose ang passport naiiba lang ang usage nun kapag naka specify sa VISA kung ano ang pinunta nya sa bansang pupuntahan nya. Ang requirements naka depende yan sa edad nang aplikante.

  16. Is there anyway that we can get a passport without NSO birth certificate? I all have other valid documents except NSO birth. May error po kasi yung birth certificate ko 😦
