I found these information concerning updating your records and registration of legal documents for the LCR of Cebu. For our Cebuano countrymen, I hope these information will be of help.

Registration of Legal Instrument Fees:

Affidavit to Use the Surname of the Father through R.A. 9255 Php 500.00
Affidavit of Acknowledgement/Affidavit of Admission of Paternity/ Affidavit Recognizing Natural Children Php 500.00
Affidavit of Legitimation by Subsequent Marriage of Natural Parents Php 500.00
Voluntary Emancipation of a Minor Child Php 100.00
Supplemental Report of Birth Php 200.00
Option/Election to Philippine Citizenship Php 2000.00
Certificate of Naturalization Php 2000.00
Affidavit or Oath of Allegiance or Wife or children of a Citizen Php 2000.00
Certificate of Legal Capacity to Marry Php 1000.00
Affidavit of Reappearance Php 1000.00
Certificate of Repatriation Php 1000.00
Authorization or Ratification of Artificial Insemination Php 1000.00
Registration of other Legal Instrument Php 500.00

REQUIREMENTS

a) Affidavit to Use the Surname of the Father (RA 9255)

1. Three (3) Original Copies (with original signature) of the Affidavit to Use the Surname of the Father (AUSF), typewritten and duly signed by the father;
2. Three (3) Original Copies of Affidavit of Consent of the Mother;
3. Two (2) photocopies of birth certificate (Form 1A) or Certificate of Live Birth of the child;
4. Any document showing clearly the paternity between the father and the child: (Strictly required when the name of the father does not appear in the birth record of the child), i.e.;
1. Employment records (with benefeciaries)
2. SSS/GSIS records
3. Insurance
4. Statement of Assets and Liabilities
5. Individual Income Tax Return (ITR) of the father
6. Others (showing relationship between father and child)

5. Three (3) Original Copies of Affidavit of Acknowledgement/Recognition duly signed by the father (Strictly required when the father does not appear in the birth record of the child);
6. Three (3) copies of Affidavit of Consent of the Child, if the child is 18 years old and above;
7. Show valid IDs of both parents (i.e. SSS/GSIS, Voter’s ID, Postal ID, Driver’s License, Passport, etc.)

b) Affidavit of Recognition (for birth records of those born before August 3, 1988)

1. Three (3) original copies of Affidavit of Acknowledgement/Recognition duly signed by the father (must be typewritten/computer printout);
2. One (1) original and two (2) photocopies of Birth Certificate (Form 1A) or Certificate of Live Birth;
3. Any document showing clearly the paternity between the father and the child, i.e.;
1. Employment records (with benefeciaries)
2. SSS/GSIS records
3. Insurance
4. Statement of Assets and Liabilities
5. Individual Income Tax Return (ITR) of the father

4. Show valid ID of the parents (i.e. SSS/GSIS ID, Voter’s ID, Postal ID, Driver’s License, Passport, etc.)

c) Affidavit of Legitimation by Subsequent Marriage

1. Three (3) original copies of Affidavit of Legitimation duly signed by the parents (must be typewritten/computer printout);
2. Three (3) original copies of Affidavit of Recognition by the father (if the father’s name does not appear in the original birth certificate);
3. One (1) original and two (2) photocopies of the Marriage Contract of the parents;
4. Certificate of No Marriage CENOMAR, from the National Statistics Office of both parents;
5. Any document showing clearly the paternity between the father and the child, i.e.;
1. Employment records (with benefeciaries)
2. SSS/GSIS records
3. Insurance
4. Statement of Assets and Liabilities
5. Individual Income Tax Return (ITR) of the father

6. Show valid ID of the father (i.e. SSS/GSIS ID, Voter’s ID, Postal ID, Driver’s License, Passport, etc.)

d) Supplemental Report of Birth

1. Any document showing clearly the paternity between the father and the child, i.e.;
1. Name of Document Owner
2. Date of Birth, Marriage or Death
3. Registry Number
4. Entry/entries omitted during the time of registration
5. Reason for omission of the entry/ies during the event
6. Purpose in supplying the omitted data

2. Two (2) photocopies of Certificate of Birth showing omitted data
Note: The one to file the supplemental report are the following:
1. Parent
2. Guardian
3. Party concerned (of legal age)

    -kailangan pa po ba ng cenomar? meron na po kasi marriage contract. Mag rerelease din po ba yung nso ng cenomar kahit nkaregistered na married na po yung pangalan namin?

      Kung Legitimation by Subsequent Marriage ang concern mo, ito ang mga requirements na kailangan mong i-attach sa affidavit:

      1. Affidavit of Paternity / Acknowledgement (Certified Photocopy)

      2. Joint Affidavit of Legitimation

      3. Certificate of Registration of Legal Instrument (Affidavit of Legitimation)

      4. Certified True Copy of Birth Certificate with remarks/annotation based on the legitimation by subsequent marriage.

      Maaari ring manghingi ng iba pang documents ang munisipyo, depende sa evaluation nila ng papers ninyo.

      Kung kasal na kayo, mag request man kayo ng copy ng CENOMAR, lalabas na din na may reported marriage na kayo.

Ng apply po ako ng AUSF sa anak ko tapos nkuha ko yung local birth cert.ng anak ko may remarks na sa ibaba nkasaad na “THE CHILD SHALL BE KNOWN AS ————na pwede na niyang gamitin ang surname ng papa niya.tapos nung kumuha na ako ng nso ang sabi wala pa daw sa data base nila pinababalik ako ngayun sa lcr .ano paba dapat ang susunud kong gagawin? Back to zero na nmn ba ako .hinahanap kasi ng nso ang tracking no.daw dahil wala pa sa data base nila

      1. Sa cebu city yun lang ang hawak ko yung local birthcert.na may nakasaad ng remarks sa ibaba.2005 pa yun sabi noon nila sa akin ok na daw yun .pero nung komuha na ako ng nso eh wala pa daw sa data base nila pinababalik nila ako ngayun sa local

  3. Good day Maam/ Sir!

    I am currently applying for a passport. I have already prepared my documents and asked for assistance from an agent. However, the agent informed me that the entries on my NSO birth certificate are not clear so I was asked to have as supporting document my certificate of live birth.
    The entries in my certificate of live birth which was the original one given when I was born are still readable. However, as per the agent the DFA office will not accept my Certificate of Live Birth since it is old already. I also presented a baptismal certificate as another supporting document.

    Is the reason they have given which is my Certificate of Live Birth being “OLD” valid so as to require me to acquire a new one issued by the Local Civil Registry? I was born in Leyte and currently residing in Cebu so it would require me a lot of time just to get a new one. Also, the said Certificate of Live Birth was honored when I presented when I took my Licensure Examination last 2011.

    I hope you can help me with this matter.

    1. Was the said document is the original or certified true copy from LCR? What you need is a certified true copt from LCR if your NSO copy is blurred or have an unreadable part. If you still have some relatives in Leyte you can ask them to request a copy of your local B.C

    1. Usually inaabot ng 3-6 mos yung proseso. pero kung may kopya ka na ng finality and endorsement ask mo na yung lokal nyo kung kelan ka puedeng mag follow up sa NSO main sa may east ave.

  4. i got a big problem with the local registry office of lahug cebu city,..i applied my birth certificate in nso via online,..well i got it on time the problem is the dfa didnt accept it because of multiple unreadable entries..its so frustrating….what can i do ?
