Registration of Legal Instrument Fees:

Affidavit to Use the Surname of the Father through R.A. 9255 Php 500.00
Affidavit of Acknowledgement/Affidavit of Admission of Paternity/ Affidavit Recognizing Natural Children Php 500.00
Affidavit of Legitimation by Subsequent Marriage of Natural Parents Php 500.00
Voluntary Emancipation of a Minor Child Php 100.00
Supplemental Report of Birth Php 200.00
Option/Election to Philippine Citizenship Php 2000.00
Certificate of Naturalization Php 2000.00
Affidavit or Oath of Allegiance or Wife or children of a Citizen Php 2000.00
Certificate of Legal Capacity to Marry Php 1000.00
Affidavit of Reappearance Php 1000.00
Certificate of Repatriation Php 1000.00
Authorization or Ratification of Artificial Insemination Php 1000.00
Registration of other Legal Instrument Php 500.00

REQUIREMENTS

a) Affidavit to Use the Surname of the Father (RA 9255)

1. Three (3) Original Copies (with original signature) of the Affidavit to Use the Surname of the Father (AUSF), typewritten and duly signed by the father;
2. Three (3) Original Copies of Affidavit of Consent of the Mother;
3. Two (2) photocopies of birth certificate (Form 1A) or Certificate of Live Birth of the child;
4. Any document showing clearly the paternity between the father and the child: (Strictly required when the name of the father does not appear in the birth record of the child), i.e.;
1. Employment records (with benefeciaries)
2. SSS/GSIS records
3. Insurance
4. Statement of Assets and Liabilities
5. Individual Income Tax Return (ITR) of the father
6. Others (showing relationship between father and child)

5. Three (3) Original Copies of Affidavit of Acknowledgement/Recognition duly signed by the father (Strictly required when the father does not appear in the birth record of the child);
6. Three (3) copies of Affidavit of Consent of the Child, if the child is 18 years old and above;
7. Show valid IDs of both parents (i.e. SSS/GSIS, Voter’s ID, Postal ID, Driver’s License, Passport, etc.)

b) Affidavit of Recognition (for birth records of those born before August 3, 1988)

1. Three (3) original copies of Affidavit of Acknowledgement/Recognition duly signed by the father (must be typewritten/computer printout);
2. One (1) original and two (2) photocopies of Birth Certificate (Form 1A) or Certificate of Live Birth;
3. Any document showing clearly the paternity between the father and the child, i.e.;
1. Employment records (with benefeciaries)
2. SSS/GSIS records
3. Insurance
4. Statement of Assets and Liabilities
5. Individual Income Tax Return (ITR) of the father

4. Show valid ID of the parents (i.e. SSS/GSIS ID, Voter’s ID, Postal ID, Driver’s License, Passport, etc.)

c) Affidavit of Legitimation by Subsequent Marriage

1. Three (3) original copies of Affidavit of Legitimation duly signed by the parents (must be typewritten/computer printout);
2. Three (3) original copies of Affidavit of Recognition by the father (if the father’s name does not appear in the original birth certificate);
3. One (1) original and two (2) photocopies of the Marriage Contract of the parents;
4. Certificate of No Marriage CENOMAR, from the National Statistics Office of both parents;
5. Any document showing clearly the paternity between the father and the child, i.e.;
1. Employment records (with benefeciaries)
2. SSS/GSIS records
3. Insurance
4. Statement of Assets and Liabilities
5. Individual Income Tax Return (ITR) of the father

6. Show valid ID of the father (i.e. SSS/GSIS ID, Voter’s ID, Postal ID, Driver’s License, Passport, etc.)

d) Supplemental Report of Birth

1. Any document showing clearly the paternity between the father and the child, i.e.;
1. Name of Document Owner
2. Date of Birth, Marriage or Death
3. Registry Number
4. Entry/entries omitted during the time of registration
5. Reason for omission of the entry/ies during the event
6. Purpose in supplying the omitted data

2. Two (2) photocopies of Certificate of Birth showing omitted data
Note: The one to file the supplemental report are the following:
1. Parent
2. Guardian
3. Party concerned (of legal age)
