Ang Philippine Charity Sweepstakes Office (PCSO) ay nagbibigay ng tulong pinansyal sa mga Philhealth at non-Philhealth members na kailangang magpa-dialysis.  Layunin ng programang matugunan ang pangangailangan sa gamot at laboratory procedures ng mga pasyente na may problema sa bato o kidney.  Dahil sa kamahalan ng pagpapa-dialysis, maraming kababayan natin ang nagtitiis sa hirap ng sakit sa bato.  Sa tulong ng PCSO, mabibigyan ng pagkakataon ang mga pasyenteng makapagpa-dialysis.

Narito ang halaga ng mga tulong pinansiyal na maaaring matanggap ng mga kwalipikadong aplikante, maging miyembro man sila ng Philhealth o hinde.

For PHILHEALTH MEMBERS:

Hemodialysis Peritoneal Dialysis
 

  • Taunang tulong pinansiyal na hindi lalagpas sa Php 84,000.00 para sa gamot (epoietin).  Ang claimant ay bibigyan ng Php 14,000.00 tuwing kakailanganing magpa dialysis hanggang maka-buo ng anim (6) na sessions.

 

 

  • Ang Z package ay may taunang tulong pinansiyal na Php 84,000 para sa gastusin sa gamot (ipoietin).  Ang claimant ay bibigyan ng Php 14,000.00 tuwing kakailanganing magpa dialysis hanggang maka-buo ng anim (6) na sessions sa loob ng isang taon.

 

  • Ang taunang tulong pinansiyal para sa mga pasyenteng nasa ilalim ng case rate ay makakatanggap ng hindi lalagpas sa Php 120,000.00 para sa dialysis solutions.  Ang claimant ay bibigyan ng Php 20,000.00 sa bawat claim hanggang maka buo ng anim (6) na claims sa loob ng isang taon.

 

Para ma-determine kung anong klaseng tulong pinansiyal ang ibibigay sa mga kaso ng Peritoneal Dialysis, kailangang mag-sumite ng certification na siyang magsasabi kung anong klaseng Philhealth benefit package ang dapat sa pasyente.

 

Ang tulong pinansiyal ay ibibigay anumang oras na kailanganin ito ng claimant.  Anim na beses maaaring mag claim ang pasyente, kailangan lang mag pakita ng certification mula sa kanyang doktor bilang patunay na kasalukuyan siyang nilalapatan ng lunas para sa sakit sa bato.

For NON-PHILHEALTH MEMBERS

Hemodialysis Peritoneal Dialysis
 

  • Taunang tulong pinansiyal na hindi lalagpas sa Php 84,000.00 para sa gamot (epoietin).  Ang claimant ay bibigyan ng Php 14,000.00 tuwing kakailanganing magpa dialysis hanggang maka-buo ng anim (6) na sessions.
  •  Tulong pinansiyal na hindi lalagpas sa Php 20,000.00 para sa Peritoneal Dialysis procedure.  Maaaring mag claim ang pasyente tuwing makalawang buwan.
  • Tulong pinansiyal na hindi lalagpas sa Php 35,000.00 para sa dialysis solutions tuwing makalawang buwan.

 Ang mga non-Philhealth members ay kailangang mag sumite ng certification mula sa dialysis center bilang patunay na hindi sila miyembro ng Philhealth.

Narito ang mga documentary requirements na kailangang ilakip ng mga aplikante sa enrolment para sa hemodialysis at peritoneal dialysis:

  1. Enrollment / Unang Claim
  • Personal appearance ng pasyente sa PCSO
  • Nasagutang kopya ng Application Form
  • Orihinal na kopya ng medical abstract na may pirma ng doktor o Nephrologist. Siguraduhing nakasulat din ang license number ng doktor sa dokumento.
  • Mga reseta or prescriptions ng doktor:
    • Para sa mga Philhealth Members:
      • Gamot para sa Hemodialysis at Peritoneal Dialysis Z.
      • PD solutions para sa Peritoneal Dialysis case rate
    • Para sa mga non-Philhealth Members:
      • PD solutions para sa Peritoneal Dialysis
    • Opisyal na quotation (computation) ng gastusin mula sa dialysis centers o ospital;
    • Pinaka huling resulta ng laboratory tests ng pasyente;
    • Certificate of Acceptance of Guarantee Letter mula sa dialysis center o ospital;
    • Certification of Philhealth Membership Status kasama ang klase ng package para sa Peritoneal Dialysis (Z or case rate);
  • Valid ID ng pasyente at ng kanyang representative para sa mga susunod na claims).

2. Sa mga susunod na claims:

Pagkatapos ng unang claim, maaari nang mag assign ng representative ang pasyente para tumanggap ng mga susunod pang claims.  Kailangan lang ipakita ng representative ang index card/client-social worker agreement na ibinigay sa pasyente nang siya ay magpa enroll para makuha ang Guarantee Letter (GL).

  • Certification mula sa kanyang doktor o Nephrologist na nagpapatunay na ang pasyente ay kasalukuyang ginagamot sa sakit sa bato, kasama ang petsa ng pinaka huling treatment.
  • Orihinal na kopya ng mga reseta o prescriptions:
    • Para sa Philhealth Members:
      • Mga gamot para sa Hemodialysis at Peritoneal Dialysis Z (EPO)
      • PD solutions para sa Peritoneal Dialysis case rate.
    • Para sa mga non-Philhealth Members:
      • PD solutions para sa Peritoneal Dialysis.
  • Index card o client social worker agreement.

I can’t help but notice this trait among us Pinoys, waiting for the last minute to get things done. Tapos kapag hindi nagawa o kaya nabibitin, nagagalit tayo sa lahat ng tao sa paligid natin. Take for example getting a passport, sometimes we get to a point that we need a passport immediately to fly out, pero may problema naman sa pagkuha ng passport, kasi delayed. Saan ka lulugar? Syempre magagalit ka. Pero kung masinop ka, since and passport is valid for 5 years, dapat 1 to 2 years ago nakakuha ka na nyan. Wala naman sigurong delay ang DFA ng 12 months. Sobra na yun kung ganun.

Ganun din sa pagkuha ng NSO certificate. Magmamadali, gusto makuha na ngayon na, kasi kailangan. Sana kinuha last week, o kaya 3 days ago bago sa deadline na kailangan. Haaay buhay. Buhay Pinoy. Not ready, floating steady, going angry….

If you get the habit of being prepared for the worst, then you will be prepared for the worst and anything less than that. It is just a matter of setting the right mind set. Never wait, wag Ningas Kugon…pangit yon, pinoy na pinoy.

Magisip ka ngayon…..kailangan ko ba ng birth certificate? Hmmmmmmmm…hindi pa….pero baka…bahala na…. MALI!

Kakailanganin mo yan….kumuha ka na ngayon, itabi mo…para handa ka…mahirap ba yon? Kasi pag kumuha ka ng passport, kailangan mo yan. Pag nag trabaho ka hihingiin yan. Sa enrollment ng anak mo, kailangan nila yan…Wag ka na maghintay…gawin mo na. Dadali buhay mo. Walang pressure.

We feel undue pressure because we position ourselves under the pressure cooker. Be wise. Bawal ang tamad sa bayan na kailangan umasenso. Make a change. Change your state of mind.

You can get your passports, NSO certificates, SSS IDs, NBI stuff, POEA clearances, and other government related docs right now. Do it now. Find the ways and the means. The best and convenient options. Hanapin mo, maraming posibleng paraan sa paligid mo.

I have applied at DFA batangas for renewal of passport last March24, 2011.. they already got an appointment system, no more walk-ins..expected date of release is May 3 which is tomorrow.. rush yon, i paid 1200 for that pero ganon katagal ang waiting period, more than a month..hopefully makuha ko bukas, pero mukhang hinde. 😦

If you know someone who is in need of medical assistance, here’s a PCSO service that you can refer to: the IMAP.

Under this program, medical assistance is given to individual patients through the issuance of guarantee letters to hospitals where the patients are confined. A guarantee letter is a certification issued to hospitals for a particular charity patient under the PCSO medical assistance program where the agency assumes the obligation of settling the cost of hospitalization, including the medicines, medical, surgical or blood supplies, and diagnostic procedures.

OBJECTIVES
General : Restoration of social functioning (Physical recovery) through medical assistance
Specific : Provide assistance for hospitalization expenses, laboratory procedures and purchase of medicines, chemo drugs, dialysis solutions, antibiotics, implants, devices and other medical needs.

DOCUMENTATION REQUIREMENTS
• Letter Request to Chairman/General Manager
• Medical Abstract
• Bill/Quotation/Costing from Hospital Pharmacy/Supplies
• Laboratory Request/Medicine Prescription
• Endorsement/Acceptance letter from Hospital Social Services/Credit Collection Office

What are the Requirements for Medical Assistance?
Request for Medicines
• Letter of request addressed to the Hon. Chairman and/or General Manager
• Original/or certified true photocopy of medical abstract duly signed by the attending physician. (Note: Include physician’s license number)
• Prescription duly signed by the attending physician with costing from the hospital pharmacy included:
In cases wherein medicines are unavailable from the Hospital Pharmacy, a Certification on unavailability of medicines from the Hospital Pharmacy must be submitted to PCSO.
• Photocopy of identification card with latest picture of the requesting person with his/her signature indicated at the back.

Request for Laboratory/Diagnostic Procedures
• Letter of request addressed to the Hon. Chairman and/or General Manager
• Original/or certified true photocopy of medical abstract duly signed by the attending physician. (Note: Include physician’s license number)
• Request from the attending physician duly signed (Note: Include physician’s license number)
• Official Price Quotation from the Laboratory Section/Department of the Hospital
In cases wherein Laboratory/Diagnostic Procedure is unavailable from the hospital, a Certification on unavailability of the procedure from the hospital must be submitted to PCSO.
• Photocopy of identification card with latest picture of the requesting person with his/her signature indicated at the back.

Request for Payment Hospitalization
• Letter of request addressed to the Hon. Chairman and/or General Manager
• Original/or certified true photocopy of medical abstract duly signed by the attending physician. (Note: Include physician’s license number)
• Statement of Account/Hospital Bill certified by the billing Officer/Credit Supervisor.
• Endorsement letter from the hospital’s Social Service if there is any, or from the Credit and Collection Officer for Pay patients.

Request for Hearing Aid
• Letter of request addressed to the Hon. Chairman and/or General Manager
• Original copy of Audiological Evaluation Report duly signed by Audiometrist
• One or two price quotations from any hearing aid centers
• Photocopy of identification card with latest picture of the requesting person with his/her signature

Request for Implants/Prosthetic Devices/Wheelchair
• Letter of request addressed to the Hon. Chairman and/or General Manager
a. implant/phosthesis request
– Original or certified true photocopy of medical abstract duly signed by the attending physician. (Note: Include physician’s license number)
b. wheelchair request
– Original or certified true photocopy of medical abstract or medical certificate with wheelchair specification signed by the attending physician. (Note: Include physician’s license number)
• Two (2) official price quotations from two (2) different companies
• One (1) whole body picture of requesting patient for request for wheelchair and prosthetic devices.

Request for Dialysis
• Letter of request addressed to the Hon. Chairman and/or General Manager
• Original/or certified true photocopy of medical abstract duly signed by the attending physician. (Note: Include physician’s license number)
• Endorsement letter from a Dialysis Center or Hospital where PCSO allots an Endowment Fund
• Official Price Quotation from the Dialysis Center/Hospital
• Certification of Acceptance from Dialysis Center/Hospital
• In cases wherein dialysis solution and/or post-operative medicines is unavailable from the hospital, a Certification on unavailability of the dialysis solution and/or post-operative medicines from the hospital must be submitted to the PCSO.
