Gone are the days when you have to call your HR to request for a copy of your PhilHealth Member Data Record (MDR) whenever you get admitted at a hospital. Other times, you would have to ask a family member to visit a PhilHealth office to request for it over the counter. That process is time-consuming and involves too many people getting access to your personal information.
Now, you no longer to need to go through that long process just to get a copy of your MDR. You can now easily and conveniently download it online, print it out, and present it to the hospital or clinic.
What is a PhilHealth MDR?
Your MDR, or Member Data Record, is required when you as a PhilHealth member or one of your declared beneficiaries avails benefits from the PhilHealth during hospitalization and confinement. Sometimes, it is also required when you are applying for a job or when you need to update your records at PhilHealth.
How do I get my PhilHealth MDR online?
Getting your MDR online is easy. Just follow these steps:
- Log-on to the PhilHealth website and click on Online Services.
2. Select Member Inquiry — it is the orange icon.
3. Register or Login to your account (if you already have an account. You will need your PhilHealth Identification Number and a password to access your account. If you have forgotten your login credentials, just click on the Forgot Password link at the lower left corner of the page.
4. Once inside your account, click on Member Information to view your MDR. You will also see the Print MDR option. You may click this link to print your MDR or download a PDF copy that you can save on your computer or mobile device for future reference.
Once you have a hard copy of your MDR, you can submit this to the PhilHealth Customer Assistance desk at the hospital or clinic where you were admitted.
You also have the option to have your MDR emailed to you. Just send a request to the PhilHealth by sending an email to firstname.lastname@example.org with your personal details (name, birthdate). PhilHealth will reply with a copy of your MDR.
