How To Renew Lost Valid and Expired ePassports

I don’t know why or how but people lose their passports all the time. I know, because I lost mine some ten years back. When we were building our house and had to apply for electricity at the Meralco, my cousin borrowed my passport as proof of identification for the application. He failed to return it to me and I completely forgot that I lent it to him. One year before it expired, my niece found it in one of their cabinets and gave it back to me. Lucky me, I guess. Otherwise, I would have to go through the pains and trouble of declaring to the DFA that I lost my passport and getting a new one. Because, yes, the DFA treats the renewal of lost e-passports as new applications. Ugh, I know.

Here’s the latest list of requirements, as published on the DFA website, for those who are thinking of renewing their lost passports (valid and expired). Read on.

Lost Valid ePassport

Applications for a lost VALID ePassport will be treated as New Application.

1. Confirmed online appointment
2. Click here to see the requirements for New Adult Applicants
3. If you can present a photocopy of your last issued passport, you no longer need to bring a copy of your PSA birth certificate.
4. Police Report in English (original and photocopy)
5. Affidavit of Loss in English (original and photocopy)
6. Penalty fee: Php 350.00

There will be an additional 15 calendar days clearing period for lost, valid epassports.

Lost Expired ePassport

This, too, will be treated as anew application.

1. Confirmed online appointment.

3. If you can present a photocopy of your last issued passport, you no longer need to present your PSA birth certificate.
4. Affidavit of Loss in English (original and photocopy)
5. Penalty fee: Php 350.00

It’s basically the same except for the Police Report, right. Take good care of your passports — keep them in a designated area, protected by a jacket or leather holder. Also, it is best to keep photos of the cover and first pages of your passport in your phone or tablet, in case you need the numbers, so you won’t have to keep retrieving your passport from your files every time.

And, never ever lend your passport to anyone (right, what was I thinking?).
