Last week, we featured the solutions to problems frequently encountered in marriage and death certificates. Today (and probably in the next two more days), we are going to focus on birth certificate problems and the PSA’s prescribed solutions for each. Remember, an erroneous birth certificate may cause delays in your transactions and the longer you delay having it corrected, the more costly it may be for you.
In this blog, we will focus on problems in the FIRST NAME field of the birth certificate. Read on.
- First name is BLURRED
- If the record of PSA is blurred, the local civil registrar shall be requested to endorse a copy of the birth certificate with clearer entry in the first name to the PSA.
- If the record of PSA and the civil registry are both blurred, a petition for correction of clerical error under the provisions of RA 9048 should be filed.
- When filing a petition under RA 9048, bring the following supporting documents:
- Certified machine copy of the birth record containing the entry to be corrected;
- Not less than two private or public documents upon which the correction shall be based like baptismal certificate, voters affidavit, employment record, GSIS/SSS records, medical records, business records, drivers license, insurance, land titles, certificate of land transfer, bank passbook, NBI/police clearance, civil registry records of ascendants.
- Notice/Certificate of Posting;
- P1,000 filing fee
- First name is MISSPELLED
- The wrongly spelled first name in the birth certificate should be corrected by filing a petition for correction of clerical error under the provisions of RA 9048.
- The petition shall be filed with the local civil registry office of the city or municipality where the birth is registered.
- Bring the following supporting documents:
- Certified machine copy of the birth record containing the entry to be corrected;
- Not less than two private or public documents upon which the correction shall be based such as baptismal certificate, employment records, medical records, NBI/police clearance, civil registry records of ascendants.
- Notice/certificate of posting
- Filing fee of P1,000.
- No First Name
- A supplemental report should be filed to supply the missing entry.
- This shall be filed with the local civil registry office of the city or municipality where the birth was registered.
- First name used is different from the first name entered in the birth certificate.
- The first name in the birth certificate shall be changed by filing a petition for change of first name under the provisions of RA 9048.
- Change of first names like Ma. to Maria should be corrected by filing a petition for change of first name under the provisions of RA 9048.
- The petitions must be filed at the local civil registry office of the city or municipality where the birth is registered.
- If the petitioner has already migrated to another place within the Philippines, the petition may be filed with the civil registry office where he or she is currently residing.
- The following documents should be submitted to support the petition:
- Certified machine copy of the birth record containing the entry to be corrected;
- Not less than two private or public documents upon which the correction shall be based like baptismal certificate, employment records, GSIS/SSS records, drivers license, bank passbook, and the like.
- Notice/certificate of posting.
- Filing fee of P3,000.
- Other documents may be required such as NBI/police clearance, civil registry records of ascendants, and other clearances.
- Proof of publication.

Pano po kung gusto papalitan yun name sa birth cert, iba po kc name na gamit ko sa mga ids, and all my docs? Pwede po ba gnon?, and how much mgastos? Req?
Magastos kung papalitan mo ang pangalan mo sa birth certificate para masunod ang pangalan na ginagamit mo sa mga IDs at documents mo. Mas madali kung ang mga IDs at documents mo ang papalitan mo to the name that is written on your birth cert.

Sa LCR (munisipyo) ng birthplace po ninyo, mag file kayo ng correction of clerical error. Sila na po ang magsasabi sa inyo kung ano ang mga requirements na dapat i-submit at kung may fees na babayaran.
magandang umaga po, magtatanung lang po sana ako if pewede bang gamitin ng bata yung apelyido ng kanyang ama sa mga school records kahit ang nasa PSA birth cert nya po ay apelyido ng kanyang ina. Sa PSA birth cert. po ng bata may remarks/annotation na “legitimated due to subsequent marriage of parents”. confused lang po kasi ako kasi sa ibang PSA naman ng ibang bata may nakalagay na “legitimated to subsequent marriage of parents and the child shall be known as___”. pero dun po sa tinutukoy ko na bata wla pong naklagay sa PSA birth cert. na “the child shall be known as”.
Sa PSA birth certificate po ba ng bata nakasulat yung legitimated due to subsequent marriage of parents”? Pwede po ninyong ipa-check sa LCR kung saan naka rehistro yung legitimation kung meron silang copy ng birth certificate na naka include yung “The child shall be known as…”
Ano po ang mga kailangang gwin sa no first name at gaano po ktagal ang processing
Supplemental Report po ang tawag sa ginagawa sa LCR kapag blank yung first name field sa birth certificate. Mag inquire po kayo sa LCR kung saan naka rehistro yung birth certificate. Mag dala po kayo ng copy nung birth certificate na blank yung first name field.
May bayad po ba kapag nagpalagay ng buong pangalan? Kasi yung akin po, walang first name tska middle name.
Kung wala pong first name and middle name, mag file po kayo ng supplemental report. Mag inquire po kayo sa munisipyo, sa LCR office, ng bayan kung saan kayo naka rehistro. Sila po ang magbibigay sa inyo ng complete list ng mga requirements at kung magkano po ang babayaran para sa supplemental report.

The name in your baptismal certificate does count as a reference of your legal name. Kung ayaw niyo na pong maabala, gamitin niyo po ang pangalan niyo as it appears in your Birth Certificate — magpa palit kayo ng IDs, etc using your PSA birth certificate as proof. But if you prefer to use your baptismal certificate name: Change of first names like Ma. to Maria should be corrected by filing a petition for change of first name under the provisions of R.A. 9048.
