Since a child’s birth certificate is populated manually and almost always, by persons other than the parents of the child, errors are bound to happen. Simple errors can be corrected by a trip to the Local Civil Registry office where the birth was registered. Other errors are more complex and will need the intervention of a lawyer for a solution.
To know whether the error on your birth certificate is simple or complex, you may refer to the list below. We lifted this from the website of lawyerphilippines.org:
Birth certificate errors that you can refer to the LCR for correction:
- Change of First Name
- First name on birth certificate is Baby Boy, Baby Girl, Boy, or Girl and the child was born 1993 onwards.
- Wrong gender or date of birth (RA 10172)
- Wrong gender checked on the birth certificate
- Day or month of birth is wrong.
- Correction of clerical error:
- Blurred first, middle, or last name
- Mother’s middle name is wrong and child’s name is correct.
- Child’s middle name is wrong and mother’s is correct.
- Misspelled first name, middle name, last name.
- Supplemental report
- No first name, no last name, no middle name if legitimate or no middle name if illegitimate and acknowledged by father.
- First name on birth certificate is Baby Boy, Baby Girl, Boy, or Girl and the child was born before 1993.
- No check mark for gender or both genders checked.
If the correction you are showing on your birth certificate does not fall under any of the above-mentioned, there is a possibility that you will need the advice of a lawyer.
Of course, to be absolutely sure, it is best that you bring the erroneous birth certificate copy to the LCR first and seek their advice. They will know best how to help you correct the errors on your civil registry document.
The fees and requirements to complete the corrections shall be determined by the LCR or the lawyer, who ever will be handling your case. Always make sure to transact only with LCR staff when having your civil registry documents assessed and processed.

Kung sigurado po kayong hindi pa na rehistro ang birth niyo, pwede kayong mag file ng late registration of birth. Mag inquire ka sa LCR kung saan naka rehistro ang birth certificate mo. Itanong mo kung pwedeng mag file ka na lang ng petition for correction of clerical error since isang letter lang naman ang mali sa last name mo. Normally kasi, pag last name ang may error, dumadaan pa yan sa court order. Pero depende na rin minsan sa recommendation ng LCR, kung simpleng error lang naman, pumapayag silang petition for correction na lang. Sila na din ang magbibigay sa iyo ng list of requirements at fees na kailangang bayaran.

Here are the steps to follow when filing a petition to correct your birth order on your birth certificate:
(a). 2 latest certified LCR copies and 2 latest PSA copies of birth certificate to be corrected.
(b). 2 latest certified copies of birth certificate of all brothers and sisters of the document owner.
(c). 2 latest original or certified copies of Obstetrical record, Medical Records, and Pre-natal Records from the hospital and/or OB GYNE.
(d). 2 photocopies of any of the following documents of the parents where all their children are indicated as their beneficiary and arranged according to birth order:
SSS
GSIS
BIR
Philhealth
Private Insurance
(e). 2 copies of valid IDs of the petitioner and the document owner and 1 copy of latest Community Tax Certificate from the place of work or residence.
(f). SPA (Special Power of Attorney). If the petitioner is abroad or sick, he/she can be represented by lawyer or his/her nearest relative (up to third degree of consanguinity).
Proceed to the LCR to file the correction.
Hello po, tanong ko lang po kung magawan ng bagong birth certificate yung papa ko kahit patay na siya. Negative result yong psa niya. Hi Mary Angelie, Sorry, I think I replied to your inquiry and told you to seek the help of the LCR — completely missing that part where you said you’ve been there already. Sorry. Yes, tama ang LCR, complicated ang case niyo dahil deceased na ang father niyo who is the owner of the birth certificate. YOu might need to seek the services of a lawyer na for this, especially since may mga discrepancies pa sa COLB niya na nasa LCR.
