As mentioned in our previous blogs, there are three ways you can have the errors in your birth certificate corrected: by filing a petition for correction, going to court, or filing a supplemental report.
Today’s blog will tackle the birth certificate errors that can be corrected by supplemental reports.
What birth certificate errors need a Supplemental Report?
- No first name, middle name, or last name (legitimate).
- No middle name (if the owner is illegitimate and acknowledged by the father).
- The first name on the birth certificate is written as Baby Boy, Baby Girl, Boy, or Girl and the child was born before 1993.
- No check mark for gender/there are check marks for both genders.
- The illegitimate child wants to use the father’s surname. Take note that this correction only involves the surname. Changing status to legitimate or illegitimate requires court order/proceeding.
How to file for a Supplementary Report
- Prepare an affidavit by indicating the missing detail entry of the birth certificate owner;
- Include the reasons why the field was left blank in the birth certificate;
- Prepare other supporting documents that could help in your petition, such as medical records, school records, etc.
Submit your petition to the Local Civil Registry Office of the city or municipality where the birth was registered. If the birth certificate owner was born abroad, he can file the petition at the Philippine Consulate where the birth was reported.

Kung nung ipinanganak ka ay kasal sa iba ang tatay mo, tama lang na apelido ng nanay mo ang nakalagay sa birth certificate mo.
Walang bisa ang kasal ng mga magulang mo dahil kasal sa una ang father mo at the time na nagpakasal sila ng mother mo.
Kung gusto mong gamitin ang apelido ng father mo, kailangan mag execute siya ng AUSF (Authority to Use Surname of Father) at i-attach ito sa birth certificate mo — bilang patunay na pumapayag siyang gamitin mo ang apelido niya.
San ko po pede ipaayos ang midle name ko na dapat Pinando eh naging Penando mali po ng isang letra tapos pati po nanay ko sa brthcertificate ko ay mali pangalan at surname.. ano po ang requirements at magkano po paayos saan ko po pede ipa ayos.. gusto ko po kc kumuha ng passport matagal na.. 😔😔
Pwede po kayong mag inquire sa LCR (munisipyo) kung saan kayo naka rehistro kung pwedeng ma correct yan through Petition for correction of clerical error. Kailangan niyo din ng birth certificate ng mother niyo dahil yun ang gagamitin na basis for her correct first name and last name.

Bale apelido ba ng mother mo ang Cabilin? At ito ang nakalagay sa birth certificate mo bilang apelido mo?
Ang parents mo ba ay kasal na ng ipanganak ka?
Kung hindi pa, kinilala ka ba ng iyong ama sa iyong birth certificate? Naka pirma ba siya?
Kung hindi kasal ang mga magulang mo nang ipanganak ka, ikinasal ba sila later on?

Kung tama ang nakasulat na pangalan at apelido mo sa records ng Civil Registry, pwede kang mag request sa kanila na i-endorse yung correct copy ng birth certificate mo sa PSA para makakuha ka ng corrected copy mula sa PSA.

Gusto ko po sana alisin un middle name ng anak ko na tulad sken,im single mom po ,at un name ng tatay,pinagawa q po kasi un birth certificate ng anak ko sa tao nag wowork po ksi aq bilang ofw at no time po ako para asikasuhin dahil nid q po bumalik agad,poblema ko po nilagayan nya ng name ng tatay at sign which is sbe q no middle initial and no fathers name ano po gagawin ko?
Kung middle name and last name po ang ipapabago ninyo sa birth certificate, kailangan niyo pong mag consult sa lawyer.
