How to Renew a Lost and Expired Philippine Passport

A basic requirement for passport renewal is the submission of the expired (or expiring passport) so the DFA can cancel its validity.  But what if the old passport has gone missing?

Here’s how you can renew a lost and expired passport:

GENERAL REQUIREMENTS:

  • Personal appearance.
  • Confirmed appointment.
  • No need to submit a passport size photo.
  • Submit same documents required for first-time passport applicants.

ADDITIONAL REQUIREMENTS in case the passport was lost and cannot be found:

  • Submit a Notarized Affidavit of Loss (with detailed explanation on when, where, and how the passport got lost).
  • If the lost passport is still valid, submit a Police Report.
  • If available, bring a photocopy of the first page of the lost passport.

Take note that there will be a 15-day clearing period prior to the processing of an application for replacement of a lost valid passport.

IN CASE PASSPORT WAS ALREADY DECLARED LOST BY THE APPLICANT BUT EVENTUALLY WAS FOUND AND RETURNED TO THE APPLICANT:

  • Submit a Notarized Affidavit of Retrieval (with detailed explanation on when, where, and how passport was found).
  • Original and photocopy of the first and last page of the passport.

IN CASE OF MUTILATED OR DAMAGED PASSPORTS:

  • Submit a Notarized Affidavit of Mutilation (with a detailed explanation of when, where, and how the passport got mutilated or damaged).
  • Original and photocopy of the first and last page of mutilated or damaged passport.

IN CASE APPLICANT WAS ISSUED A TRAVEL DOCUMENT:

  • Original Travel Document
  • Submit a Notarized Affidavit of Explanation Regarding Issuance of Travel Document (with a detailed explanation of when, where, and why the applicant was issued a travel document).
  • Original and photocopy of the first and last page of canceled passport (if the passport is still in the possession of the applicant).

It is not advisable to pose as a first-time passport applicant if you lost your passport.  The DFA will see your previous passport details anyway and you will be required to submit the same documents (affidavit of loss, etc.) as listed above.  Your appointment will be wasted and you may run the risk of being placed under questionable applications.

    1. PWede naman na po kayong mag renew ng passport ngayon. Dalhin niyo na lang yung old passport when travelling to a country where you need the approved visa reflected in your old passport.

    Tanong ko lang ano dapat ko gawin tagal ko na sana magrenew un nga lang hirap makakuha ng appointment po., gusto ko po sana change status at surname using my husbands surname ano po ba dapat dalahin po NSO MC lang po ba?       Yes, kung gusto mong ipabago na ang last name mo sa passport mo, you need to bring a copy of your PSA birth certificate and your PSA marriage certificate.

      Regarding the best time to secure an appointment, according to the DFA, 9am and 12noon ang best time get an appointment online kasi those are the times when the system cleans the dates of appointments that were not confirmed.

  4. ask ko lang po sir kc naiwala ko pssport ko nung march 27, 2018 lang kaya nag pa affidavit of loss na ako saka police clearance kasi wala talaga nagreport kahit sa FB ko until now pero nung april 2 i go to the DFA ASEAN kc kalo ko tumatanggap sila ng walk in para sa mga lost passport pero sayng lang punta ko kc need appointment at ngaun napakhirap mag pa appointment pero thanks God naka kuha ako ng appointment april 7 sa ali mall.Pero pinaka problem ko po sa 15 days clearing period nila kc 15 dyas plus 7 days expidite 22 days or more pa need ko na po kc passport ko before april 21 kasi mag eexam po ako sa HRD KOREA EPS topik 2018 once a year lang po iyon sayang po ang review pagod mag register kung d ko makukuha ang passport ko before 21 need advice what to do.Pls. nag inquire na ako POEA if pde ako mag file affidavit of loss ng passport ko para makapag exam lang sabi ng POEA hindi daw pls, help me.

    My passport is expired last year (July 2017) and also marital status is changed from single to married. Does my renewal will be applied for new passport?

      PWede ka nang magpa renew ng passport mo.

      Since wala namang civil status sa passport, wala kang kailangang ipabago.

      Ang mga female passport holders lang na married na ang may option to change their last name from their maiden last name to their married last name. Option lang ito, hindi sila mandated magpalit ng apelido kahit kasal na sila.

      1. do we still need an appointment or pwede ng dumirecho? may nabasa kasi akong column pag lost valid passport pwede ng dumirecho sa ASEANA, no need an appointment. paki clear naman. salamat 🙂

  8. What if lost expired passport more than 15 years same pa din ba requirement as new and ano mga additional requirements?
