Your PSA birth certificate is a primary requirement when applying for a passport with the DFA. Without it, it would be nearly impossible to get the rest of the application process done.
Unfortunately, there are some people who really do not have their civil registry documents on file at the PSA. When they request for a copy, they receive a Negative Certification, indicating that they do not have any records or theirs may have gotten lost during or after the war.
How do you apply for a passport if you do not have a birth certificate?
Let us show you how:
If you were born in or after January 1, 1950, you need to submit the following:
General Requirements:
- Personal appearance at your chosen DFA branch.
- Confirmed appointment (done online at www.passport.gov.ph/appointment)
- Accomplished application form.
- Valid picture ID with photocopy.
- Supporting Documents
In case of NO BIRTH CERTIFICATE:
- Apply for the delayed registration of birth at the local civil registry office at the applicant’s place of birth.
- Submit the following documents:
- Authenticated Negative Result of Birth Certificate from the PSA.
- Supporting public documents with the correct date and place of birth such as:
- Form 137
- Voter’s registration record
- Baptismal certificate with readable dry seal.
- National Commission on Muslim Filipinos (NCMF) certificate with photo and readable dry seal (for Muslim applicants).
If you were born in or before December 31, 1949:
General Requirements:
- Personal appearance at your chosen DFA branch
- Confirmed appointment (done online at www.passport.gov.ph/appointment)
- Accomplished application form
- Valid picture ID with photocopy
- Supporting documents
In case of NO BIRTH CERTIFICATE:
- Certificate of Non-availability of Record from the Philippine Statistics Authority.
- Notarized Joint Birth Affidavit of Two Disinterested Persons
- Any public document with the correct full name, and date and place of birth such as:
- Baptismal Certificate with readable dry seal
- National Commission on Muslim Filipinos (NCMF) Certificate with photo and readable dry seal (for Muslim applicants).
If you are unable to secure a copy of your birth certificate from the PSA, proceed right away to the LCR of the city or municipality where you were born. There are a multitude of reasons why your birth records may not be available at the PSA, you just need to find out why so you would know what to do next. The LCR would be able to tell you what you need to do in order to have your birth records documented, or corrected, or reconstructed, whatever the case may be.

Yes, pwede naman ang out of town processing ng birth certificate correction or late registration. Pwede kayong mag file sa city hall kung saan kayo currently nakatira ngayon.

Mam, ang nanay ko hindi nman pinayagan sa cityhall sa Marinduque mag pa late register. ang sabi pa ng city hall wala daw clang say sa ganyan dapat talaga dun xia pumunta ng Mindoror.. pinapunta pa tlaga sa Mindor ang matanda, take note 77 na po siya..kaya hnd po totoo yan na pede yang out of town processing….from our experience lang po.

Yes posible pa din. Kung ano ang details na nakalagay sa birth certificate mo, yun ang susundin ng DFA sa passport mo. Since wala namang parents’ information na inilalagay sa passport, wala namang kaso ung one missing letter sa pangalan ng father mo.
Better still to have it corrected.

Kung wala pa po kayong rehistro (kailangan po ninyong makasiguro na wala pa talaga kayong rehistro), maaari kayong magpa late registration of birth.
Kung hindi na po kayo makakabalik sa lugar kung saan kayo pinanganak, pwede na din po sa probinsya o city kung saan kayo nakatira ngayon. Out of town late registration of birth po ang mangyayari para sa inyo.
Iyon lang po ang paraan para magkaroon kayo ng NSO birth certificate na kailangan ninyong i-present pag nag apply kayo ng passport.

NSO (or PSA) documents po ang requirement ng DFA.
You can try asking DFA po kung papayag sila na LCR copy muna ang i-present ninyo.

Hello po, yung lola ko po 1947 pinanganak meron naman sya b. cert ng LCR pero may mga maling info although nakita ko na meron din syang lumang affidavit about dun sa misspelled name nya na ginamit nya before to claim yung sss ni lolo. pwede na po ba to for passport application or kailangan ko po

Kung meron na syang birth certificate dati, hindi mo na siya kailangang ipa late register.
Define mo kung ano ang correction o mali sa birth certificate niya. Misspelled name ba, mali ba ang date of birth. Ang lahat kasi ng corrections sa birth certificate ay may karampatang solusyon. PWede mo itong itanong sa LCR.

Ang nire-require ng DFA ngayon is PSA na ang seal ng birth certificate (di na NSO).

Hinde. Ire-require ng DFA na ipa-correct mo muna ang gender mo bago ka nila i-accommodate for passport application.
Pwede mong ipa-correct ang gender mo sa LCR o munisipyo kung saan ka naka register. Hindi na nito kailangan ang court order or hearing.

Yes kailangan ninyo silang ipa rehistro sa munisipyo ng birth place nila. Kung masyado namang malayo ang birth place nila at mahihirapan nang mag byahe, pwede din kayong magpa late register sa munisipyo ng bayan kung saan sila nakatira na ngayon. Out of town late registration ang tawag dun.
