“Noong ‘80s, nakakuha ako ng kopya ng PSA (formerly NSO) birth certificate ko. Ngayon, nag request ulit ako, sabi sa akin ‘No Name’ na daw ako! Bakit nawala yung record ko eh meron na nga dati?”
This is a common confusion among us Pinoys when we are told that we “have a problem with our birth certificate”. When we hear the word “no”, we automatically think that we do not have a record with the PSA.
We received the above question from a follower and took it upon ourselves to clarify the matter for everyone’s benefit. It is important that we are all aware of the differences in our concerns with our birth certificates so we would also know the most effective and efficient solution we could apply.
Below is a summary of the most common birth certificate issues encountered by Pinoys and the prescribed solution for each:

| 1. No Record of Birth Certificate | The PSA does not have a copy or record of your birth certificate because:
A. Your parents failed to register your birth at the LCR. Or B. The LCR where your birth was registered failed to submit your birth certificate to the PSA for certification. C. You were born during or shortly after WW2 when most birth records were destroyed or misplaced. |
For scenarios A and C: Submit an Affidavit for Delayed Registration of birth at the LCR of your birthplace. Read this article for the complete list of requirements for Delayed Registration.
For scenarios B: Advise the LCR to endorse your Certificate of Live Birth to the PSA for proper certification.

| 2. Misspelled First Name | One or two letters were mistyped that may result in one of two things:
A. Correcting the mistyped letters will confirm your real name or b. Correcting the mistyped letters will give you a new name (e.g. Rachelle is the correct spelling but your name is typed as Rochelle). |
For scenario A: File an Affidavit for Clerical Error at the LCR where your birth was registered.
For scenario B: The LCR will advise you if you still need to undergo court proceedings as well as other fees you may need to pay (as the case may be treated as “change of name” instead of “clerical error”.)

| 3. “Baby Boy” and “Baby Girl” in the first name field | The phrases “Baby Boy” and “Baby Girl” were typed in the first name field and is now considered the person’s first name. |

A. If the petitioner is SINGLE:
B. If the petitioner is MARRIED:
File your petition for correction at the LCR where your birth was registered. |
| 5. No Name on Birth Certificate | The first name field in the child’s birth certificate is blank. | File a supplemental report at the LCR of the child’s birthplace to supply the missing entry. |
| 6. Wrong Gender | Gender written in birth certificate is the opposite of the owner’s actual gender. | This is considered a clerical error and maybe rectified by filing a Petition for clerical or typographical error. |
| 7. Birth Certificate is unreadable or blurry | Entries in your birth certificate are hard to read because the texts are smudged or the prints have faded over time. |

| 8. Problems with Entries in Birthplace field | A. Only the name of the hospital is indicated in the birthplace field.
B. There are no entries in the birthplace field. C. The birthplace written in your birth certificate is incorrect |

Parents and guardians are strongly encouraged to double-check all entries in a child’s birth certificate before submitting the document for registration. Once this is made official by the LCR, you might have a difficult time applying corrections on entries overlooked prior to submission; some errors might even entail cost.

Ipa correct niyo po sa GSIS ang records ninyo and use your PSA birth certificate and PSA marriage certificate as proof of your real birth year. Ipa correct niyo na din yung records niyo government agency kung saan kayo nag trabaho.

Paano pong “nagkamali”? Nagkamali po ba ng spelling o totally ibang apelido ang nailagay sa birth certificate niya? Kung nagkamali lang ng spelling, baka pwede pang Petition for Correction of Clerical Error yan sa LCR kung saan naka rehistro ang birth mo.

Subukan niyo po mag request ng copy sa LCR ng bayan kung saan kayo pinanganak. Ipa check mo muna doon kung wala kang rehistro. Kung may mahahanap silang record ng birth mo, ipa-endorse mo na sa PSA para magkaron ka ng PSA birth certificate. Pero kung wala silang mahanap na records mo, ibig sabihin hindi ka talaga registered. Sasabihin na din sa yo sa LCR kung ano ang requirements for late registration of birth.

Hello po! Yung mother in law ko po, (70 y.o)kukuha sana ng passport, 1st time nya, pero mali ang date of marriage sa marriage contract nya from PSA copy. Ano po kaya ang mga hakbang na dapat nyang gawin? Sana po ay masagot. Maraming salamat po.
Gusto pa po ba niyang gamitin ang married name niya sa passport niya? Kasi pwede naman na birth certificate na lang niya ang ipakita niya sa DFA at yung maiden name niya ang gamitin niya sa passport.

Kung may copy na ang mother niyo ng corrected birth certificate niya, pwedeng gamitin yun as basis for the correction of her name’s spelling sa birth certificate ninyo. Nag tanong ka na ba sa munisipyo ng birthplace mo kung may clear copy sila ng birth certificate mo? Dahil kung meron, all you have to do is ask them to endorse the clear copy to PSA para magkaroon ka ng clear copy ng birth certificate mo.
How about death certificate? Kumuha po ako ng kopya ng death cert ng father ko sa civil registry pero wala na silang naibigay. Before po kasi nakakuha na mother ko pero nawala ang kopya kya kumukuha ulit ako. Pero sabi wala po silang mahanap na ganong tao. We’re not sure po kasi kung anong exact date alam lang namin month and year. Wala po bang way pano mahanap yun sa PSA? Pag nagbigay ka naman ng wrong date, negative ibibigay ng PSA. Waste of money lang. Ano po bang way para ma-sure sana yung date at di masayang ang pagkuha ng death cert ng father ko.

Basta’t tama ang spelling ng pangalan niya at alam ninyo yung month and year, lalabas at lalabas yan sa PSA. Basta’t nakakuha na kayo ng kopya sa munisipyo before.

Unang-una, falsification of public documents ang ginawa nila sa pag lagay ng fake na date of marriage at place of marriage. Krimen yun.
Pangalawa, kung gusto nila itong ipa-correct, kailangan nilang dumaan sa korte at mag hire ng abogado. Malaki ang posibilidad na makita na naglagay sila ng hindi totoong information sa birth certificate ng bata.

Kung ito po ang case, hindi na po matatanggal yung middle name ng bata. Kung babae po yung anak, mababago lang yan kapag kinasal na siya (dahil magpapalit na siya ng apelido). Pero as of now, wala na pong magagawa para ma-correct yan.

Pwede ninyo itong ipa correct sa munisipyo kung saan naka rehistro ang anak mo. Aks the LCR kung pwedeng Petition of Correction of Clerical Error ang gawin (para mura lang at mabilis). Bring a copy of your PSA birth certificate para may basis sila ng correct middle name na dapat ilagay sa birth certificate ng anak mo.

May dalawa pong problema ako sa documents ng magulang ko, una po mali yung age na nakasulat sa marriage certificate nila, 5 years ang pagitan nila pero sa nkalagay sa marriage certificate nila nkalagay ay pareho silang 25 years old.
Pangalawang po is unreadable po ang birth certificate ng daddy ko, at nka register pa sa npaka layong lugar eh senior napo at mahina na ang kalusugan pra sa biyahe. Pano po bang mas madali or recommend niyo po? Reconstruction of the documents or pede pa siya i pa delayed birth registration nalang kahit na my records nmn siya sa psa kaso sobrng unreadable lang.

Ipa-reconstruct ninyo ang birth certificate niya kung unreadable na ito. Huwag kayong mag papa late register for him kasi madodoble lang ang registration niya, lalo lang kayong magkakaproblema.
Yung age nila sa marriage certificate, pwedeng ipa correct yun through Petition for Correction of Clerical Error, using his and your mother’s birth certificates as basis for the correct ages. However, kung sinadyang gawing mali ang ages nila when they got married (for whatever reason), baka magka problema kayo (sa legitimacy ninyo as their children and legitimacy ng kasal nila).
So ayusin niyo muna yung birth certificate niya kasi yun ang gagamitin ng LCR na basis for the correction of their ages in their marriage certificate.

PSA ba mismo ang nag prescribe ng fees (at hindi fixer)? Kung PSA ang nagsabi sa iyo na may fee, para sa processing yan at papers na gagamitin for the endorsement. Syempre po, merong cost ang production kaya may fees tayong kailangang bayaran.
Please make sure you are transacting with a government employee and you are paying at the office’s cashier and you are issued a government receipt. Huwag na huwag po kayong makiki transact sa fixer kahit sabihin pa nila na mas mapapadali ang proseso if you do it through them.

Pwedeng Petition for Correction of Clerical Error na lang sa LCR kung saan naka register ang birth certificate ng husband mo. Magdala na lang ng copy ng birth certificate ng mother niya para may basis sa correct spelling ng middle name niya.

Mag request ka ng Negative certificate from the PSA. Pag naka kuha ka na nito, dalhin mo sa munisipyo kung saan ka nakarehistro at mag request ka ng copy ng birth certificate mo.
Kung meron silang maibibigay sa iyo na kopya, mag request ka ng endorsement to the PSA para magawan ng certified copy ang birth certificate mo.
Kung wala, mag file ka ng late registration of birth. Ang list of requirements at fees ay ibibigay sa iyo sa munisipyo.
Dalhin mo din ang papers ng adoption mo para meron silang basis.

You can have your birth certificate corrected through a Petition for Correction of Clerical Error and you can have this done at the city hall or municipal hall where your birth was registered. Bring copies of your parents’ birth certificates and a copy of their marriage certificate.

Married ba ang parents mo? Ang Gardiola ba ang maiden last name ng mother mo bago siya kinasal?
Kung yes ang sagot mo sa dalawang tanong ko sa taas, pwede kang mag file ng Petition for Correction of Clerical Error at mag present ka ng birth certificate ng mother mo kung saan Gardiola ang maiden last name niya na naka sulat.

If the copy that they have is also blurry, you will have to request for a reconstruction of your birth certificate and have them endorse this to the PSA so the PSA can give you an authenticated copy.

My dad was trying to apply for Philippine passport. He has been using the same birthday since he was working. He did not have birth certificate or baptismal certificate kasi nasunog daw po ang munisipyo that time at wala silang kopya. He just used an affidavit that shows the birthdate he’s slwaus known. My mom went to LCR and filed for Delayed Registration. Nung naipasok na po lahat sa PSA sa manila, nung tubusin ni mama yung birth certificate, yung pong lumabas na birth certificate is yung sa kapatid nya. Parang lumalabas po na 5 months lang ang pagitan nila (my dad is dec 1949, my aunt is may 1950). We do not know how to correct it kasi everytime mag request po si mama, yung sa kapatid ng dad ko yung lumalabas na narehistro.

Ask the LCR of the city hall (or municipal hall) about reconstruction of birth certificates.

Kung Baby Boy lang (walang Edzel) and born before 1993:
a. A supplemental report has to be filed at the Local Civil Registry of the city or municipality where the birth was registered. If the child was born abroad, the report must be filed with the Philippine Consulate of the country where the birth was reported. In case the child is now residing in the Philippines, the report may be coursed through the Department of Foreign Affairs, Office of Consular Affairs.
b. Apart from the owner of the birth certificate, the following may also file on behalf of the owner: owner’s spouse, children, parents, brothers, sisters, grandparents, guardian, other person duly authorized by law or by the owner of the document sought to be corrected.
d. Bring an affidavit indicating the entry missed in the registration and the reasons why there was a failure in supplying the required entry. The LCR may require other documents as deemed necessary.
If born 1993 onwards:
a. A petition for change of first name under R.A. 9048 must be filed with the local civil registry office of the city or municipality where the birth is registered. In case the owner has transferred to an area that is far from his place of birth (within the Philippines), the petition may be filed with the civil registry office of his current residency. If the owner was born abroad, he must file the petition at the Philippine Consulate where his birth was reported.
b. Apart from the owner of the birth certificate, the petition may also be filed by the owner’s spouse, children, parents, brothers, sisters, grandparents, guardians, other person duly authorized by law. The same are applicable if the owner of the record is a minor or physically or mentally incapacitated.
c. The petitioner must bring the following documents upon filing:
Certified machine copy of the birth record containing the entry to be corrected;
Not less than two (2) private or public documents upon which the correction shall be based like baptismal certificate, voter’s affidavit, employment record, GSIS/SSS record, medical record, business record, driver’s license, insurance, land titles, certificate of land transfer, bank passbook;
Notice / Certificate of Posting;
Filing fee of Three Thousand Pesos (P3,000) if in the Philippines and $150.00 or equivalent value in local currency if filed abroad;

BUT, if the name is BABY BOY EDZEL, your husband will have to file for a change of name and that process will undergo court proceedings and will need the services of a lawyer.

Please read the guidelines below:
If the child was born before 1993
a. A supplemental report has to be filed at the Local Civil Registry of the city or municipality where the birth was registered. If the child was born abroad, the report must be filed with the Philippine Consulate of the country where the birth was reported. In case the child is now residing in the Philippines, the report may be coursed through the Department of Foreign Affairs, Office of Consular Affairs.
b. Apart from the owner of the birth certificate, the following may also file on behalf of the owner: owner’s spouse, children, parents, brothers, sisters, grandparents, guardian, other person duly authorized by law or by the owner of the document sought to be corrected.
c. If the owner of the record is a minor or physically or mentally incapacitated, the petition may be filed by his spouse, or any of his children, parents, brothers, sisters, grandparents, guardians, or persons duly authorized by law.
d. Bring an affidavit indicating the entry missed in the registration and the reasons why there was a failure in supplying the required entry. The LCR may require other documents as deemed necessary.
If the child was born in 1993 onwards
a. A petition for change of first name under R.A. 9048 must be filed with the local civil registry office of the city or municipality where the birth is registered. In case the owner has transferred to an area that is far from his place of birth (within the Philippines), the petition may be filed with the civil registry office of his current residency. If the owner was born abroad, he must file the petition at the Philippine Consulate where his birth was reported.
b. Apart from the owner of the birth certificate, the petition may also be filed by the owner’s spouse, children, parents, brothers, sisters, grandparents, guardians, other person duly authorized by law. The same are applicable if the owner of the record is a minor or physically or mentally incapacitated.
c. The petitioner must bring the following documents upon filing:
Certified machine copy of the birth record containing the entry to be corrected;

Nangyari yan dahil nagpa late register siya kahit na may original record na siya — ibig sabihin, na-rehistro na siya noon pa. Pero siguro dahil may nakitang mali sa birth certificate niya, ang ginawang solusyon ay late registration. So lumalabas, dalawa ang record niya. Invalidated yung late registration niya. Ang nakuha mong birth certificate sa Pasay ay yung original at unang registration niya kung saan ang pangalan niya ay Segondo.
Subukan ninyong ipa-cancel yung original na record para yung late registration ang magamit na niyang record niya. Dadaan sa korte ito (cancellation of records) at ang desisyon ay manggagaling sa korte.

Maaaring mapalitan ang apelido ng bata kapag kinasal na kayo ng father niya. Pwede kang mag file ng Legitimation due to Subsequent Marriage.
Kung hindi naman kayo kasal pero kinikilala ng ama ang kanyang anak, pwede kayong mag apply ng AUSF (Authority to Use the Surname of the Father). Ito ang katibayan na pumapayag ang ama na gamitin ng anak ang apelido niya sa birth certificate.

Kailangan mong mag file ng supplemental report sa LCR. Pero hindi aabot ng isang buwan ito. This will take about 2 to 3 months. Magpa appointment ka na sa DFA ngayon pa lang para habang inaasakiso mo ang supplemental ng bc mo, at least naka reserve ka na ng slot mo sa DFA.

Proceed to the municipal hall or city hall where your birth certificate was registered and ask them to check if your birth certificate on record bears the correct gender. If it does, request that the properly filled out birth certificate be endorsed to PSA (NSO) for certification.
In case wala talagang nakasulat na gender kahit sa copies ng LCR, you’ll need to have the birth certificate corrected through a petition for correction of clerical error. You can have it facilitated at the LCR too, but may take 3 to 6 months before you get the corrected copy.

Subukan niyo munang pumunta ng PSA to get a copy of Negative Certification. Pag meron ka na nun, pumunta kayo sa munisipyo kung saan naka rehistro ang father mo. Ask kung meron silang copy ng birth certificate ng father mo. Kung meron: mang hingi ka ng endorsement para ma forward yung record ng father mo sa PSA. Kung wala: mag file ka ng late registration of birth. Ang list of requirements, ibibigay na sayo sa LCR.

Mag inquire ka muna sa LCR kung saan naka register ang birth certificate mo para malaman mo kung pwedeng Petition for correction of clerical error lang yan. Ibig sabihin, aalisin lang yung o sa dulo ng name mo to make it Ronald.
However, may malaking posibilidad na padaanin yan sa isang court order kasi ang name na Ronald ay valid name din. Madalas kasi , pumapayag lang silang ituring na clerical error ang misspelled first name kung hindi valid yung misspelled name (or a name that does not make sense).. Since Ronald is a valid name, pwedeng lumabas na change of name ang mangyayari sa iyo (at hindi correction). Pag change of name ang nangyari, dadaan sa court hearing yan.
May option ka din to adopt your true name (Ronald) and just change your documents and IDs with the correct spelling of your name. Mas mura yun dahil affidavit lang ang i-execute mo sa mga offices (like SSS, Pag-IBIG, banks, etc) para baguhin nila ang pangalan mo.

We are not sure how much LCRs charge for this type of correction; below are the fees charged by the Manila City Hall which you may use as reference:

Bakit daw po “not valid” ang result?
Nasiguro niyo na po ba na talagang hindi kayo na-register before?
Kasi, kung magpapa late register kayo ngayon, tapos lalabas later on na naka rehistro pala kayo at hindi lang agad nahanap ang records niyo, mado-doble po ang registration niyo.
Just the same, narito po ang proseso ng late registration pero ang advice po namin sa inyo ay siguraduhin niyo muna na talagang wala kayong registration before bago kayo mag file ng late registration:
If the person is less than eighteen (18) years old, the following
shall be required:
1. Four (4) copies of the Certificate of Live Birth duly
accomplished and signed by the proper parties;
2. Accomplished Affidavit for Delayed Registration at the back of
Certificate of Live Birth by the father, mother, or guardian,
declaring therein, among other things, the following:
a. name of child
b. date and palce of birth
c. name of the father if the child is illegitmate and has
been acknowledged by him;
d. if legitimate, the date and place of marriage of parents;
e. reason for not registering the birth within thirty (30)
days after the date of birth
3. In case the party seeking late registration of the birth of an
illegitimate child is not the mother, the party shall, in addition to
the foregoing facts, declare in a sworn statement the recent
whereabouts of the mother.
4. Any two of the following documentary evidences which may show
the name of the child, date and palce of birth, and name of mother
(and name of father, if the child has been acknowledged):
a. baptismal certificate;
b. school records (nursery, kinder-garten, or preparatory;
c. income tax return of parent/s;
d. insurance policy;
e. medical records; and
f. others, such as barangay captain’s certification.
Affidavit of two disinterested persons who might have
witnessed or known the birth of the child.

Nagpakasal ba sila pagka panganak sa yo at nag file ba sila ng Legitimation Due To Subsequent Marriage?

Pwede kang magpa-correct sa munisipyo kung saan naka rehistro ang birth certificate mo. Pwedeng i-correct ang spelling ng middle name mo through a Petition for Correction of Clerical Error. Magdala ka ng PSA birth certificate ng mother mo para may basehan sila ng tamang spelling ng middle name mo.

Gudmorning po, kumuha po ako birthcertificate ko, cristina po ang pangalan ko, ang lumabas po cristina, tapos bday ko po, june 5,1978,,bakit po july 15,1978 sa psa.
Kinasal po ako nung april 14,1997,,nagpasa po ako ng birthcertificate ko nung time na yun, cristina po at june 5 1978 ang nakalagay, tapos po October 1 2019,,nagrequest po ako ng psa sa savemor bakit mali po yung lumabas
