The Quezon City Hall Series: How to File an Affidavit for Clerical Error and Change of First Name

A most common issue encountered by Filipinos with their birth certificates are misspelled first, middle, or last names.  These can go from just a single misplaced letter to an entire unknown name that somehow made it to the person’s birth certificate by mistake.  If you need to have your name corrected or changed, you may do so at the city or municipality where your birth was registered.

Here is the list of requirements and fees when filing for an affidavit for clerical error and change of first name at the Quezon City Hall.  This is also otherwise known as Republic Act 9048.

Requirements for Clerical or Typographical Error:

  1. Certified true machine copy of the Certificate sought to be corrected.  Make three copies.
  2. Documents showing the correct entry/entries upon which the correction shall be based such as Baptismal Certificate, Voter’s ID, School Records, GSIS records, SSS records, medical records, business records.
  3. Other relevant documents as the Registrar may require.
  4. Filing Fee: PHP1,000 and additional service fee for Migrant Petitioner of PHP500.00.

Requirements for Change of First Name/Nickname:

  1. Certified true machine copy of the Certificate sought to be corrected.  Make three copies.
  2. Clearance from the following authorities (these are MANDATORY):
    • Clearance from Employer or Certification that the applicant has no pending case with the company (if employed).
    • Affidavit of No Employment (if the applicant is not employed).
    • NBI or PNP Clearance
    • Documents showing the correct entry/entries upon which the correction shall be based, such as: Baptismal Certificate, Voter’s ID, School records, GSIS records, SSS records, medical records, business records.
    • Other relevant documents as the Registrar may require.
  3. Filing fee: PHP3,000.00 and additional service fee for Migrant Petitioner of PHP1,000.00.

Applicants must proceed to the Civil Registry Department for further instructions.

  1. Hello po, yung mother ko passed away na pero need po namen ng birth cert nya na tama yung name. Sa birth cert po kasi nya Lucia ang first name nya pero ever since po and ginagamit nya is Melinda, lahat po ng documents nya pati marriage contract nila ng father ko Melinda nakalagay. Ano po procedure for this and ano po requirements ang need? Thank you po!

    1. Subukan niyo pong mag inquire sa LCR kung pwedeng ipa-correct ang birth certificate ng isang deceased person. Ang mahirap po kasi diyan, hindi lang po basta mali ang spelling ng pangalan niya. Ibang iba po kasi sa nakasulat sa birth certificate versus sa ginamit niya sa mga IDs niya ang other documents. May malaking chance po na kailanganin niyo din ang services ng isang lawyer dahil hindi po simpleng correction yan. Change of first name po ang mangyayari kung sakali. Iba po yun sa correction.

  2. Mag papa clerical error po sana ako kasi mali yung name and even apilyedo ng parents ko And yung saakin po ay mali din apilyedo. Nanghihingi po sila ng reuqirements na mga documents na tama apilyedo ko e lahat po ng requirements ko syempre susundan B.C ko. Pano po kaya ito.

  3. Good day! Ask ko lang po paano kung yung surname ko is gusto kong palitan. Like for example is yung surname ko is sa father ko and ang gusto kong mangyari is tanggalin yun and gawin kong apelido lang yung sa mother ko. Pwede po ba ito? Kase ganon yung sa brother ko, bali ako na lang di pa napapalitan ng apelido.

      Pwede naman pero kailangan itong dumaan sa court proceeding, lalo na kung legitimate child ka at dapat talaga na apelido ng father mo ang gamit.

      Mag inquire ka sa isang family lawyer kung papano ang pinaka mabilis na proseso para magawa ito at kung magkano ang magagastos.

  6. Hello po ask ko lang ipapa correct ko kase po name ng mother and father ko sa nso ko, tapos yung iba ng requirments ko wala na yung original xerox/photo copied na lang matatanggap kaya yun as suporting documents?

  7. Good day po. Gusto ko lang itanong, yung misspelled ba na first name sa birthcertificate sa clerical error sya or sa change of name na? Yung mother ko kasi ang ginagamit nyang first name is Nelsy pero ung sa birthcert nya Nelsie ang spelling. Paano po yun? Napansin ko din kasi na mas malaki ang fee sa change of name. 😦

    1. I update mo lahat ng requirements mo sunod sa PSA(NSO) B.C mo. Ok lang naman na hinid kasal ang mga magulang na register naman na nakasunod ka sa name ng father mo eh. Ayaw mo ba?

      1. Pano po kung ang gamit mo na apelyido na nakalagay sa id’s, school record at diploma mo eh sa apelyido ng tatay mo pero pagkuha mo sa bc mo eh ang naka register eh apelyido pala ng nanay mo.. Pano po kayang gagawin don? Salamat sa sasagot..
