Have you ever had to process a correction on your or a family member’s birth certificate?

I know how inconvenient it is to have to process such corrections, especially if you are pressured by time.  How much more if you are not fully aware of the processes you have to follow and the documents you need to submit?  Hassle talaga.

So to help ease your burden a bit, I am sharing with you the following information lifted from the Philippine Statistics Authority website, on applying for corrections on NSO Birth Certificates.

Based on RA No. 10172, ito ang mga cases ng correction na hindi na kailangan ng Court Order or Judicial Proceedings:

Clerical or Typographical Errors on:

  1. Pagpapalit ng first name or nickname
  2. Pagpapalit ng araw at buwan ng kapanganakan
  3. Pagpapalit ng kasarian

Kung ang case mo ay isa sa tatlong nabanggit, hindi mo na kailangan ng court proceedings.   Pwede na kayong magpa-correct ng entries by simply executing an affidavit kung saan naka-saad ang mga errors or ang mga detalye na kailangan mong papalitan.  Importante lang na ang error na pinapa-correct or detalye na pinapapalitan natin ay malinaw na pagkakamali sa typing or data entry lamang.

Kasama ng affidavit, kailangan din natin mag- submit ng mga sumusunod na documents:

  1. Certified true machine copy of the certificate or of the page of the registry book containing the entry or entries sought to be corrected or changed;
  2. At least two (2) public or private documents showing the correct entry or entries upon which the correction or change shall be based;
  3. Other documents which the petitioner or the city or municipal civil registrar or the consul general may consider relevant and necessary for the approval of the petition.

As soon as the details have been corrected, kayo ay i-isyuhan ng LCR ng Finality Report.  Kapag may kopya na kayo ng Finality Report, maaari na kayong mag request ng First Corrected Copy ng inyong birth certificate sa NSO East Avenue or any NSO outlet.  Ito na ang maayos, malinaw, at tamang kopya ng inyong birth certificate.
