For those living in Manila, planning to visit the Manila City Local Civil Registrar for their birth certificates or other registration concerns, I shall make a series on the requirements and protocols you might need to know.
Let’s start with this:
REQUIREMENTS FOR CORRECTION OF CLERICAL ERROR IN THE MIDDLE NAME OR MOTHER’S NAME OF DOCUMENT OWNER (MARRIED) UNDER RA 9048
1. 2 latest certified/local copies and 2 latest NSO copies of birth certificate to be corrected
2. 2 latest certified copies of birth certificate/death certificate of mother. If certified of no record was issued, please submit birth certificate or marriage contract of mother’s 2 brothers or sisters.
3. 2 latest certified copies of marriage contract of parents. If certification of no record was issued, please submit birth certificate of documents owner’s brother or sister at least two (2).
4. 2 latest certified copies of marriage contract of document owner
5. 2 copies of baptismal certificate
6. 2 copies of school records (Elementary and High School either F-137/138 or Certification) or College (TOR)
7. 2 certified copies of voter’s registration record/voters affidavit. (COMELEC)
8. 2 copies of valid ID of the petitioner and the document owner and 1 copy of latest community tax certificate from the place of work or residence
9. Other documents which the office may consider relevant and necessary for the approval of the Petition. GSIS/SSS Record, Diploma (Elem., HS or College/Vocational), Medical Record, Business Record, Service Record, Insurance, Certificate of Land Title, Passbook, etc.
10. SPA (Special Power Attorney), If the petitioner is not the document owner like auntie, uncle godparents, client, friend, officemate etc.
REMINDERS:
1. All civil documents (Birth, Marriage and Death) to be submitted should be the latest certified
local copy or security paper from NSO
2. After the compliance of the requirements, please proceed to the information counter and get a number for the pre-interview and bring the original copies of the supporting documents (Personal Records) (Only with complete requirements are allowed for pre-interview)
3. Steps to follow will be provided after the Final Interview
4. Processing of the petition is four (4) months. Will commence on the date the petition will be receive by this Office
5. Payments are as follows: Registration Fee (P 1,000.00)/Certified Xerox Copy (P230.00) and Transmittal Fee (P210.00) and Additional Payment of P30.00 (document with supplemental report)

Mali po ang middle name ko sa bc ko. Pati din maiden name at pangalan mama ko mali po. Pangalan dn ng papa k mali dn po.. Ano po dpat kung gawin.
Sa ganyan kadaming mali ng dokumento mo mag consult ka na sa abogado. Bka pwedeng ipa cancel na lang ang rehistro mo na yan.
Sir/Mam, paano po kung mali yung middle name na inilagay sa birth certificate nung manugang ko? Kc po lola pa nya ang nagsulat nun ng iparehistro sya jn sa civil registrar? Adult na po sya ngaung malaman nya na magkaiba yung middle name na nsa birth cert.nya at ginagamit nya? Ano po pwedeng gawin dun? At magkano po ang magagamit pra maayos po un?-Mrs.Austria
Ipa pa ayos nya yun sa munisipyo kung saan siya naka register. I e evaluate ng KCR officer yung dapat na gawing proseso sa kanya.
Wala pong nationality na nakalagay sa information ng parents ko sa aking birth certificate. Sabi po ng DFA ayusin ko daw muna na yung missing entries sa birth certificate ko bago magka passport pero di naman sinabi saan at paano ko gagawin yun. Sana matulungan ninyo ako
Sa munisipyo kung saan ka naka register mo ito aayusin ang gagawin ay supplemental report kasi kulang ng details.
HI Good day ask ko lang wala po kasi akong name sa birth certificate ko from nso pero meron akong name sa birth certificate ko in local ano po ba magandang gawin para magkaroon ng name yung birth certificate ko may mga affidavit po ako na nagpapatunay na name ko po yun thanks ?
File ka ng supplemental report of first name sa munisipyo kung saan ka naka register.

I think you just have to photocopy it but always bring the original for reference.

hello po, ano po ba ang magiging middle name ng anak pag hindi kasal ang parents at naka apelyido sa mother ang bata kahit acknowledge ng father ang sa birthcertificate, kasi pinalagyan po ng middle name doon sa local civil registrar noong ni register sa PASIG CITY po naka register at ang naging middle name po ng bata ay middle name din po ng mother, pwede po ba ipa change o ano ang magiging middle name or middle initial ng bata? eh hindi daw pwede e leave blank yong middle name? kelangan po kasi sa passport may middle name at iba pang mga papers, since ang bata ngayon 6 yrs old na pwede pa ba yon bagohin? ask ko lang po, thank you.
Kasal na ba yung magulang nung bata? Pwede na itong ipa acknowledge of paternity sa tatay at magamit ang surname nya.

Meh tanung lang ako regarding sa Passport ko. ksi hindi kasal ang nanay at tatay ko, so from elementary up to High School ang gamit kong apelyido ay sa nanay ko, pero wala akong Middle name sa NSO ko. Pro sa lahat ng mga papers ko meron akong Middle name gnagamit. So ngayon po Kumuha ako ng Passport. hindi nla pinagamit ang aking MIddle name kasi yun yung nsa NSO ko. Ngayon Nagapply ako abroad, required pala n dapat meh Middle name yung Passport ko. Gusto ko po sana ipabago at mapalagyan ng Middle name yung Passport ko. Paano po ba ang mdaling process. Kasi po mejo Malaki na ang aking gastos. Maraming salamat po. and God Bless. Hello.Pls advise po if it is possible to get a copy of Affidavit to use the surname of the father as well as the acknowledgment of paternity from the civil registry office in manila. I don’t have the original documents po kasi coz they were taken by NSO when we have filed for annotation for changing my daughter’s surname. Tnx
Pwede naman sigurong mag request pa. Para saan mo ba gagamitn? Kasi everytime naman na kukuha ka na sa NSO ng copy yung may annotation na ang ibibigay nila sa iyo eh.

All you need naman is the NSO annotated copy nung document.
Thank you for your prompt reply, and may i inquire once i ask for endorsement on my case, how long does this take? Will the endorsement be on the day i go to your office so that all i would need is a follow up from NSO? and how long will it take before NSO can act on this and eventaully i will have a record with them?

There is no window for prompt processing on your case it’s up to the LCR officer if they can expedite the process.

Ang problema ko po ay ang name at middle name ko sa Birth Certificate ay iba sa ginagamit ko ngayon. Ang nasa birth certificate kop o ay Daniel Sambrano Corpuz subalit ang ginagamit ko po ay Danilo Cister Corpuz. Ang Cister po ay apelyido ng Lola ko. Ang mother’s maiden name ko po ay Maria Cister Sambrano. Noong po ako ay nagsimulang mag-aral ay Danilo Cister Corpuz na po ang gamit ko pati nung ako’y nag-asawa (marriage contract po naming ay Cister din middle name ko at ang name ng nanay ko po doon ay Maria Cister (dahil po ang sabi ko ay hnd po kasal ang lolo at lola ko subalit wala na pong makakapagpatunay dahil 1924 po pinanganak anany ko at wala na pong records sa LCR at NSO. Ang middlename ko pong Cister ay gamit ng mga anak ko, voters registration, drivers license, insurance, land title at iba pang mahahalagang dokumento na kung susundin kop o ang birth certificate ko ay mahahagip po lahat yon. Ang tanong ko po ay kung pwede ko pong maapply ng change name and middle name sa LCR at gamitin ang Danilo Cister Corpuz, na hindi na kelangan pumunta sa korte. Kelangan kop o ng advice mo kung paano po ang pinaka mainam na paraan at gawin ko. God bless po!!
Puede mo naman itong i file ng kaukulang proseso pero depende ito sa evaluation ng LCR officer kung ano ang dapat. Kung mag advise sila na kailangan itong idaan sa korte yun ang dapat mong gawin.

Mali yan balik ka kung saang DFA mo na process ang passport mo para ipa correct yan. Hindi mo ba yan nakkita nung ipabasa ng encoder sa iyo ang mga detalye mo?

Hi, is this what i need to accomplish if my name (middle name) in my son’s birthcertificate is wrong (my husband wrote my father’s middle name). There are so many types of affidavit of discrepancy i dont know wc to file. Thankyou!
To be correct, you need to go to the local civil registrar where you will file this correction. The LCR will tell you the proper requirements you will need to have it corrected.
