10. Hello.Pls advise po if it is possible to get a copy of Affidavit to use the surname of the father as well as the acknowledgment of paternity from the civil registry office in manila. I don’t have the original documents po kasi coz they were taken by NSO when we have filed for annotation for changing my daughter’s surname. Tnx

Will the endorsement be on the day i go to your office so that all i would need is a follow up from NSO? and how long will it take before NSO can act on this and eventaully i will have a record with them?

  14. Hi master,

      1. it was añana when i read it ..di ko poh alam kung bat gnito nngyari megamall ako ng apply …bblik nlng ako dun for correction ..hindi na nman poh cguro kelangan mg pa sched ng appointment dba ?

  17. Hi, is this what i need to accomplish if my name (middle name) in my son’s birthcertificate is wrong (my husband wrote my father’s middle name). There are so many types of affidavit of discrepancy i dont know wc to file. Thankyou!

    1. To be correct, you need to go to the local civil registrar where you will file this correction. The LCR will tell you the proper requirements you will need to have it corrected.

    1. Christian, the LCR procedures are mostly the same with minor variances like fees and small requirements. If you are int he province, check out your LCR expecting nearly 90% of the same procedures posted in this blog for Manila LCR.
